Holy Cross Hospital Health Centers are primary care medical centers that provide affordable services to only the uninsured in the area. Patients who come to the health center receive care from skilled clinicians who perform follow-up care for emergency room and hospital visits, physical exams and chronic disease management. Staff members speak English, French and Spanish, and interpreting in other languages is available.
As the leading safety net provider in Montgomery County, Md., Holy Cross Hospital provides through two clinics access to prenatal, primary, emergency and hospital care for those unable to pay — one of the clinics is capable of handling at least 10,000 patient visits per year.
Both clinics provide:
- Health Education — The center periodically hosts health fairs that cater to the multicultural community and cover diabetes, nutrition, high blood pressure, cardiovascular wellness and weight control.
- Physical Exams and Lab Work — Medical staff complete thorough assessments and will order lab work as needed through on-site labs or through a specialized referral at Holy Cross Hospital.
- Diabetes and Cardiovascular Management — Doctors and nurses specializing in the prevention and management of diabetes and cardiovascular disease provide assessment, education and specialized care plans for patients.
- Pharmacy — On-site pharmacies support the clinics, but, if staff is unable to provide medications from their pharmacies, they help patients to obtain no-cost and low-cost medications.
- Assessments and Referrals — Trained licensed clinical social workers and patient navigators are part of the program, assisting patients with mental health assessments and social service referrals.
