St. John Providence Health System opened the St. John Community Health Center on the eastside of Detroit in 2000 to address the primary care needs of the uninsured with a special focus on persons with chronic disease. The majority of patients at the Health Center are African-American, between the ages of 19 and 64, and are 200 percent or below the poverty level. The Health Center primarily provides chronic disease management, with the top diagnoses being hypertension, diabetes and asthma. Approximately 80 percent of patients are diagnosed with two or more chronic diseases.

Services provided include:

  • Preventive care and health education.
  • Management of acute and chronic diseases.
  • Lab services.
  • Social services.
  • Pelvic exams and pap tests.

Patients enrolled at the health center are able to receive diagnostics and pharmacy for free or at a reduced rate. In addition, this is assisted by an active pharmacy assistance program which helps patients secure medications from pharmacy manufactures at no cost to them. Many of the Health Center's patients are on five or more medications for chronic disease management.

The St. John Community Health Center had 1,900 visits in FY2010, and averages 100-150 new patients a year.
